Webinar Description: Children's Care Management is available to assist and empower so many young adults and families. Providers benefit as well since our services increase communication amongst all the different systems of care, providers, school staff and social services involved.

• Participants will have a clear understanding of some of the Medicaid changes and the purpose of Children's Care Management Services.
• Participants will increase communication with other providers, school and social service staff.
• Participants will understand eligibility for services and how to qualify those in need.
• Participants will learn how to connect young adults and families with Care Management Services.

Speaker Bio: Michelle Varvaro is responsible for Community & Provider Relations with CHHUNY: Children's Health Home of Upstate NY.

Michelle holds an MA in Rehabilitation Counseling and an MA in Special Education. She has spent 30 years working in Mental Health and Substance Abuse programs in Rockland, Westchester and Bronx Counties.

Always a strong believer in using community resources, Michelle participates in multiple groups and collaboratives throughout NYS and runs the CHHUNY Community Youth Collaborative for the Capital Region of NY.
